Fare Refund Request

If you feel that you were charged an incorrect fare from Uber using Innisfil Transit, please first report this using the Uber app by selecting "Review my fees and fares" at the bottom of your Trip Receipt.

If this does not provide you with an appropriate resolution within 3-5 business days, then please fill out this form.

Please note that the Town is unable to directly provide you with a refund, but can assist you in contacting Uber to receive a refund from Uber

  • Photocopy of license
  • Photocopy of recent utility bill
  • Municipal tax bill
  • Other official document containing your name and address

Upon receiving your submission, the Town will contact Uber with your request. A refund, if deemed appropriate, will be deposited directly to your payment method on file within your Uber account. It may take 3-5 business days for Uber to process any refund.

Personal information collected on this form is pursuant to the Municipal Freedom of Information and Protection of Privacy Act, R.S.O. 1990, Chapter M.56, as amended, and will be used to provide to Uber in order to assist you with requesting a fare refund for an Innisfil Transit trip.
